PARTICIPANT INFORMATION
HAIR SERVICE REQUESTED
WIG FITTING (HAIR LOSS CLIENTS ONLY)
Available only for individuals experiencing hair loss due to Alopecia,
Cancer Treatment, Lupus, Trichotillomania, Burns, Medical Conditions,
or Other Hair Loss Disorders.
HEALTH, ALLERGY & SCALP DISCLOSURE
WIG FITTING ACKNOWLEDGEMENT
I understand that wig fittings provided during HAIRitage Day are intended
to help individuals experiencing hair loss feel confident, supported,
and empowered.
- Wig fittings do not constitute medical treatment.
- Wig availability may be limited.
- A wig fitting does not guarantee receipt of a custom wig.
- Additional measurements, consultations, or follow-up appointments may be required.
- The Walker Foundation reserves the right to determine eligibility for wig assistance programs.
CONSENT FOR HAIR SERVICES
I voluntarily authorize licensed professionals and trained volunteers
participating with The Walker Foundation to provide hair services and/or
wig fitting services to myself or my child.
LIABILITY RELEASE
I acknowledge that participation may involve risks including but not limited to:
- Skin irritation
- Allergic reactions
- Scalp sensitivity
- Discomfort from styling products
- Reactions to adhesives, wig caps, or hair products
I release and hold harmless The Walker Foundation, HAIRitage Day,
volunteers, cosmetologists, barbers, braiders, wig specialists,
sponsors, venue partners, and affiliated organizations from liability
arising from participation, except in cases of gross negligence or
willful misconduct.
PHOTO & MEDIA RELEASE
I authorize The Walker Foundation to photograph and/or record myself
or my child during HAIRitage Day for educational, promotional,
fundraising, website, social media, and marketing purposes.
ACKNOWLEDGEMENT & SIGNATURE
I certify that I am the participant or legal parent/guardian of the participant
and that I have read, understood, and voluntarily agree to this waiver and consent form.
